REGISTRATION FEES
ALL FOOTBALL PLAYERS:
$240.00 Uniform Fee: $160.00
ALL CHEERLEADERS:
$175.00 Uniform Fee: TBA (we are trying to have it cost no more than $250.00)
$100.00 Deposit // Balance must be paid by April 18, 2008 (**100.00 of this fee is Non-Refundable or Non-Transferable**) No paperwork accepted without fees.
$100.00 deposit non-refundable/non-transferable due at the time of registration. Entire Registration/Uniform fee after May 16, 2007 is non-refundable/non-transferable.
FOOTBALL/CHEER APPLICATION - EASTBAY & NATIONAL
BIRTH CERTIFICATE (Veteran players/cheer on file - 2007 Season only)
MEDICAL FORM - Dated Jan 1, 2008 or later.
PICTURE - 2X2 OR 2X3 WALLET SIZE SCHOOL PICTURE - NO HATS, SCARFS OR DARK GLASSES. NO POLAROID'S ALLOWED, HEAD AND SHOULDER ONLY.
COMPLETE REPORT CARD - September 2007 - June 2008 or SCHOLASTIC ELIGIBILITY FORM - If report card is lost, incomplete or grades below 2.0. If a scholastic eligibility form is turned in as scholastic eligibility, a progress report will be MANADATORY by National Pop Warner Standards as well as East Bay Warriors.
ZERO TOLERANCE POLICY
SCHOLASTIC ALL AMERICAN FORM - Must be completed before the first day of practice. This form is to recognize academic achievement and shall be filled out or your child will be dropped from the program.
EQUIPMENT RELEASE FORM - 10 hours of conditioning must be completed before equipment is distributed.
FEES VOLUNTEER HOURS/EQUIPMENT/FUNDRASING - $60.00 - Each family must complete 30 hours to receive refund. Time cards will be distributed at the beginning of the season. Find a list of jobs in your parent package. Refunds will be issued at the end of the season by mail.
I, as parent/guardian and/or signer, understand and agree that East Bay Warriors do not give out a participation award and that my child must complete the whole entire season with the team/squad that they are assigned to in order too receive an award at the end of the year banquet. This may or may not include going to the National Championships.
